DIRECTIONS FOR FILLING OUT AND SUBMITTING VARIOUS SHWAY FORMS:
In order to fill out standard events (weekly events, bimonthly and monthly events)at a particular parish, one form is needed :
1. Read SHWAY Handbook. (Handbooks may be obtained at parish offices where SHWAY is based or at www.shway.org) and call Coordinator (808-672-8669)
2. Fill out General Waiver Registration (GWR) form for parish event. (if event is at St. Jude, you would fill out St. Jude GWR form note: GWR forms apply until July 1, 2010
In order to attend Youth Nights or special on site events(events that take place on parish grounds), two forms are needed.
1. If GWR form hasn’t been filled out, fill out and submit GWR form for parish event, after reading the SHWAY Handbook.
2. Fill out Youth Night/Special On site event events form and submit by date mention on form. Please call Coordinator at 808-672-8669 if you are going turn in forms after due date or on the day the event occurs.
In order to attend 2010 SPAM 5 retreat, two forms are needed.
1. If GWR form hasn’t been filled out, fill out and submit GWR form for parish event, after reading the SHWAY Handbook.
2. Fill out SPAM 5 form Checks made payable to St. Jude Catholic Church) and submit with money to Keith Febrero by date mention on the form. Please call Coordinator at 808-672-8669 if you are going turn in forms after due date or on the day the event occurs
