Option 2: Submit forms during CYYAM onsite hours
Option 3: Approach Keith at the piano after the 11:00am Mass

Step 3: MAKE AN APPOINTMENT WITH THE COORDINATOR OF YOUTH & YOUNG ADULT MINISTRY (CYYAM)
